ABSTRACT

Se presenta un caso de flutter auricular, con bloqueo auriculoventricular 2:1, asociado a hidrops en uno de los fetos de un embarazo gemelar triple, pesquisado por taquicardia fetal y confirmado con ecocardiografía a las 26 semanas de gestación. Se administró digoxina a la madre, sin exito, por lo que agregamos flecainida al decimo tercer dia de tratamiento, logrando conversión a ritmo sinusal y regresión del hidrops dentro del útero. Se discute la utilidad del flecainide como primera linea para este tipo de pacientes y la necesidad de profilaxis antiarritmica postnatal, considerando la favorable evolución en este periodo.

ABSTRACT

Presentamos un caso de flutter auricular, con bloqueo auriculoventricular 2:1, asociado a hidrops en uno de los fetos de un embarazo gemelar triple, pesquisado por taquicardia fetal y confirmado con ecocardiografía a las 26 semanas de gestación. Iniciamos digoxina transplancetaria sin éxito por lo que agregamos flecainide al décimo tercer día, logrando conversión a ritmo sinusal y regresión del hidrops in útero. Se discute la utilidad del flecainide como primera línea para este tipo de pacientes y la necesidad de profilaxis antiarrítmica postnatal, considerando la favorable evolución en este período.

ABSTRACT

Presentamos un caso clínico de diagnóstico antenatal de síndrome de tallo corporal. El ultrasonido de un feto de 16 semanas evidenció un defecto amplio de la pared toracoabdominal con evisceración del corazón y órganos abdominales, corroborados en el exámen anatomopatológico. Los hallazgos sugieren un subgrupo de síndrome del tallo corporal, debido a su asociación con alteraciones morfológicas de órganos internos. Se realiza una revisión de la literatura y se discute el manejo perinatal

ABSTRACT

Alcohol ingestion is considered as a possible pathogenic agent for Berger's disease, since IgA mesangial deposits have been described in liver cirrhosis. Aiming to assess this issue, 28 patients with Berger's disease (BD) and 40 patients with other glomerulopathies (NBD) were subjected to an enquiry about alcohol ingestion. Data was corroborated with 21 closed relatives of BD patients and 34 relatives of NBD patients. No differences were observed in report alcohol intake between BD patients and their relatives, however relatives of NBD patients understimated their alcohol intake. No differences in alcohol intake, either self reported or reported by relatives, were observed between BD and NBD patients. It is concluded that no differences in alcohol intake were observed between patients with Berger's disease and subjects with other glomerulopathies
